Case Manager Shelf Meal Order in Vandalia OH

Below you will be able to request Shelf Stable Meals for your consumer online. This is the only way that we will be accepting shelf meal requests. Please fill in your name as the requesting case manager, the client's first and last name, their funding program, member ID, the amount of meals you are requesting and then you will be able to select the meals on behalf of the client.

Deadlines for Shelf Meals are as follows:

October 31, 2018 submission for November 2018 delivery.

November 30, 2018 submission for December 2018 delivery.
